This is a sterling silver round container that's either a pill box or perhaps a snuff box. It has four holes in it for “breathing” and a slide out hinge for opening and closing and access to pills or whatever it is intended for. It is hallmarked “Silver“ on the back and has etched in designs on the front that look like stalks or ferns.
